Change Request Procedure
The Data Policy & Practice (DPP) change request process intends to create an opportunity for alerts and communication about changes planned to systems with potentially cascading effects. Such changes would include changes to business rules impacting the interpretation of underlying data; changes to system configuration, impacting reporting and/or automation; Introduction of new systems or data sets, impacting reporting and/or automation.

Completing the form: the form should take about 5 minutes to complete. It is vital that the change be categorized as “Data Policy & Practice” and that the subcategory is set as either DPP committee, finance, staff, or student data steward group. These items are in bold below.
Search requester: your name (if submitting via the Learning Hub, this field should auto-populate)
Subject: brief description of the change
Change Type: Minor
Status, Priority: leave default
Impact: leave as default (low) unless a major change is requested with medium or high impact
Group: leave blank
Agent: leave blank
Description: detailed description of the change, including any links to supporting documentation
Planned start & end date: enter your best estimate, including time (you may just enter 12:00 AM)
Department: select your department
Category: Data Policy & Practice
Sub-Category: select the group for whom the change could have impact; if more than one, select All DPP Members; this selection is what determines the teams channel on which the change request will be posted
Maintenance window: leave blank
Complete the planning section, including reason for the change, brief description of impact, brief rollout plan (a single line will suffice), and backout plan (if none is needed include na)

2. Select Approver CAB: On the next screen select the approvers, by selecting the data steward group that corresponds with the subcategory selected. (Finance, Student, Staff, DPP Committee, All) - this selection will determine who receives the request email. Approval must be requested by a licensed Freshservice agent. Data Team & IT can assist with this.
